BariatriCare Subscription Policy
As an annual subscriber to the BariatriCare program, I am committing to a $99.99 charge for the first twelve months while I am enrolled in the program. Paying this initial payment is my acknowledgement that I am requesting to become a subscriber to the program with the effective date being the date the first charge is processed on my credit card and the instructions for the subscription and passwords are provided to me. My credit card will be charged the non-discounted price of $179.99 every twelve months until I cancel my subscription as noted below.
Cancellation Policy - The subscription will begin on the date of the first charge to the credit card, and will continue to be charged to the same credit card on the annual anniversary date of the first charge. I understand that in electing to subscribe to the BariatriCare Program, there are no refunds on any period during which I am a subscriber. In order to cancel my subscription, I will notify Mind Body Health Services, Inc, d/b/a Dr. Connie Stapleton by email at info@bariaftercare.com before the end of the subscription period. In doing so, my subscription will be effectively canceled on the next annual anniversary date after the notifying email is sent by me and I understand that no refunds will be issued.
